[codex] Use local environment for user shell commands (#28163)
## Why User shell commands still read the legacy turn cwd and session shell even though execution context is now owned by selected turn environments. App-server also defines `thread/shellCommand` as a local-host escape hatch, so it must use an available local environment even when a remote environment is primary. ## What changed - Add `ResolvedTurnEnvironments::local()` to find the selected local environment. - Resolve the user shell command cwd and shell from that local `TurnEnvironment`. - Emit the standard `shell is unavailable in this session` error when no selected local environment or resolved local shell is available. - Add an integration test covering `/shell` without a local environment. ## Test plan - `just test -p codex-core user_shell_command_without_local_environment_emits_error`

[codex] Bind shell snapshots to retained thread environments (#28421)
## Why Shell snapshots are currently session-scoped even though shell and cwd are properties of a selected turn environment. That makes snapshot refresh depend on separate session-cwd plumbing, prevents retained environments from retaining their snapshot work, and can make snapshot construction use a different shell than command execution. This follows #27955 by making the retained thread-environment service own environment snapshot lifecycles. Session configuration remains the requested selection state, while `ThreadEnvironments` remains the source of successfully resolved environments. ## What changed - Configure the shell-snapshot builder before initial environment resolution. - Start each local environment snapshot task when its `TurnEnvironment` is built and retain that shared task while environment ID and cwd still match. - Inherit retained environment snapshots into spawned child threads. - Carry the selected `TurnEnvironment` through shell runtimes so snapshot construction and command execution use the same environment-specific shell and cwd. - Load project instructions and warm plugins/skills after initial environment resolution. - Continue decoding invalid UTF-8 instruction files lossily without emitting a startup warning. - Keep requested selections in `SessionConfiguration`; failed or duplicate resolutions only affect the resolved environment snapshot. ## Validation - `cargo check -p codex-core --tests` - `just test -p codex-home instructions` (6 passed) - Focused environment, instruction, shell-snapshot, and user-shell tests (84 passed) - Focused shell-snapshot, user-shell, and unified-exec tests (126 passed; two event-timing tests passed on retry)

feat(core): add metadata field to ResponseItem (#28355)
## Description This PR adds an optional `metadata` field to `ResponseItem` for Responses API calls. Only mechanical plumbing, no actual values populated and sent yet. Turns out just adding a new field to `ResponseItem` has quite a large blast radius already. This change is backwards compatible because `metadata` is optional and omitted when absent, so existing response items and rollout history without it still deserialize and requests that do not set it keep the same wire shape. For provider compatibility, we strip out `metadata` before non-OpenAI Responses requests so Azure and AWS Bedrock never see this field. My followup PR here will actually make use of it to start storing and passing along `turn_id`: https://github.com/openai/codex/pull/28360 ## What changed - Added `ResponseItemMetadata` with optional `turn_id`, plus optional `metadata` on Responses API item variants and inter-agent communication. - Preserved item metadata through response-item rewrites such as truncation, missing tool-output synthesis, compaction history rebuilding, visible-history conversion, rollout/resume, and generated app-server schemas/types. - Strip item metadata from non-OpenAI Responses requests while preserving it for OpenAI-shaped requests. - Updated the mechanical fixture/test construction churn required by the new optional field.

[codex] simplify shell snapshot ownership (#27756)
## Why Shell snapshot lifecycle state was split between `Shell` and `SessionServices`: `Shell` carried the receiver while session code exposed and forwarded the raw sender. That coupled shell identity to mutable snapshot state and made refresh, inheritance, and file lifetime harder to reason about. ## What changed - make each `Arc<ShellSnapshot>` represent one cwd-specific snapshot generation - store the active generation in `SessionServices` with `ArcSwapOption` - have construction start the background build and expose only a cwd-validated snapshot path - use `ShellSnapshotFile` ownership to delete snapshot files automatically - pass snapshot paths explicitly to shell runtimes instead of storing snapshot state on `Shell` - preserve inherited and in-flight generations by pinning their `Arc` while they are in use ## Test plan - `cargo check -p codex-core --lib` - `just test -p codex-core 'shell_snapshot::tests'` - `just test -p codex-core shell_command_snapshot_still_intercepts_apply_patch` - `just test -p codex-core shell_snapshot_deleted_after_shutdown_with_skills`

Support plaintext agent messages (#27830)
## Why Multi-agent v2 `send_message` deliveries already reach the receiving model as typed `agent_message` items with encrypted content. Child-completion notifications are generated by Codex itself, so their content is plaintext and previously fell back to a serialized JSON envelope inside an assistant message. With plaintext `input_text` supported for `agent_message`, both delivery paths can use the same model-visible type while preserving explicit author and recipient metadata. ## What changed - add plaintext `input_text` support to `AgentMessageInputContent` and regenerate the affected app-server schemas - preserve `InterAgentCommunication` as structured mailbox input instead of converting it to assistant text - record delivered communications as typed `agent_message` history items - persist a dedicated rollout item so local delivery metadata such as `trigger_turn` remains available without leaking into the Responses request - reconstruct typed agent messages on resume and preserve fork-turn truncation behavior - remove request-time assistant-content parsing - preserve plaintext and encrypted inter-agent deliveries in stage-one memory inputs - normalize and link plaintext and encrypted agent messages in rollout traces without treating inbound messages as child results - cover the real MultiAgent V2 child-completion path end to end with deterministic mailbox synchronization ## Verification - `just test -p codex-core plaintext_multi_agent_v2_completion_sends_agent_message` - `just test -p codex-core input_queue_drains_mailbox_in_delivery_order record_initial_history_reconstructs_typed_inter_agent_message fork_turn_positions_use_inter_agent_delivery_metadata` - `just test -p codex-memories-write serializes_inter_agent_communications_for_memory` - `just test -p codex-rollout-trace agent_messages_preserve_routing_and_content sub_agent_started_activity_creates_spawn_edge` - `just test -p codex-rollout-trace agent_result_edge_falls_back_to_child_thread_without_result_message` - `just test -p codex-protocol -p codex-rollout -p codex-app-server-protocol`

Add spans to turn lifecycle gaps (#27623)
## Why Codex app-server latency traces do not granularly cover turn task startup and inter-request handoffs. These spans help attribute time across task execution, startup prewarm, in-flight tool completion, and rollout persistence. ## What changed - Add `session_task.run` spans around task execution and `session_task.flush_rollout` around flushing pending conversation transcript writes to durable storage - Add `regular_task.prepare_run_turn` around regular-turn startup (Send the `TurnStarted` event, reset turn-specific reasoning state, and resolve any startup prewarm) - Add `startup_prewarm.resolve` around waiting for background session prewarming to finish, fail, time out, or be cancelled - Add a function-level trace span around draining in-flight tool calls (Wait for tool calls to complete, record tool result in conversation history, and other bookkeeping) ## Verification Trigger Codex rollout and observe new spans are included

Add app-server background terminal process APIs (#26041)
## Summary Codex Apps needs app-server as the source of truth for chat-started background terminals instead of guessing from local process trees. This PR adds experimental v2 APIs to list and terminate background terminals for a loaded thread using app-server process ids, so clients can manage background terminals without local PID discovery. ## Changes - `thread/backgroundTerminals/list` returns paginated background terminal records with `itemId`, app-server `processId`, `command`, `cwd`, nullable `osPid`, nullable `cpuPercent`, and nullable `rssKb`. - `thread/backgroundTerminals/terminate` terminates one running background terminal by app-server `processId` and returns whether a process was terminated. - Background terminal list and terminate operations use unified-exec process manager state as their source of truth.

feat: count V2 concurrency by active execution (#26969)
## Why Multi-Agent V2 concurrency should count active non-root turns, not resident or durable agent threads. The limit is intentionally best effort: admission checks are synchronous, but concurrent successful checks may overshoot slightly. ## What changed - Keep one root-derived execution limit on the shared `AgentControl`. - Count active V2 subagent turns with an RAII guard owned by `RunningTask`. - Check capacity before spawning or starting an idle agent, including direct app-server `turn/start` submissions. - Preserve queued delivery for agents that are already running. - Exempt automatic idle continuations so `/goal` work is not dropped when capacity is temporarily full. - Keep root and V1 turns outside this limiter. ## Test coverage - `execution_guards_count_active_v2_subagent_turns` - `execution_guards_ignore_root_and_v1_turns` - `v2_nested_spawn_checks_shared_active_execution_capacity`

[codex] Add turn profiling analytics (#26484)
## Summary Add flat profiling fields to `codex_turn_event` so analytics can explain where turn wall-clock time is spent without changing tool execution behavior. The profile reports: - time before the first sampling request - sampling time across all attempts and follow-ups - overhead between sampling requests - time blocked in the post-sampling tool drain - time after the final sampling request - sampling request and retry counts ## Implementation - Extend the existing turn timing state with constant-memory phase accounting and one RAII phase guard. - Observe sampling and the existing post-sampling drain only at turn orchestration boundaries. - Keep tool runtime, tool futures, response item handling, and turn lifecycle values unchanged. - Add the profiling fields directly to the existing analytics turn event without changing app-server protocol or rollout persistence. - Use the existing turn `status` to distinguish completed, failed, and interrupted profiles. Exact sampling/tool overlap is intentionally omitted because measuring tool completion accurately would require hooks in the tool execution path. ## Validation - Add app-server end-to-end coverage for a single-sampling turn with no blocking tool work. - Add app-server end-to-end coverage for `request_user_input` blocking followed by a second sampling request. - CI is running on the PR; tests were not executed locally per repository guidance.

cli: add package path from install context (#26189)
## Why Codex package installs include helper binaries in `codex-path`, such as the bundled `rg`. Package-layout launches should add that directory before user commands run, but standalone launches were missing it while npm launches only worked because `codex.js` had its own legacy `PATH` rewrite. That made npm and standalone package behavior diverge. Shell snapshot restoration can also reset `PATH` after runtime setup. Any package-owned `PATH` prepend has to be recorded as an explicit runtime override so shells, unified exec, and user-shell commands keep access to `codex-path` after a snapshot is sourced. ## Repro Before this change, a curl-installed package could contain `rg` under `codex-path` but still fail to put it on `PATH`: ```shell mkdir /tmp/test-codex-curl curl -fsSL https://chatgpt.com/codex/install.sh \ | CODEX_HOME=/tmp/test-codex-curl CODEX_NON_INTERACTIVE=1 sh /tmp/test-codex-curl/packages/standalone/current/bin/codex exec \ --skip-git-repo-check 'print `which -a rg`' find /tmp/test-codex-curl -name rg ``` The `which -a rg` output omitted the packaged helper even though `find` showed it under `/tmp/test-codex-curl/packages/standalone/releases/.../codex-path/rg`. The npm install path behaved differently only because `codex-cli/bin/codex.js` had legacy `PATH` rewriting: ```shell mkdir /tmp/test-codex-npm cd /tmp/test-codex-npm npm install @openai/codex ./node_modules/.bin/codex exec --skip-git-repo-check 'print `which -a rg`' ``` That printed the npm package's `vendor/<target>/codex-path/rg` first. This PR moves that behavior into Rust-side package launch setup so curl/standalone and npm/bun launches agree without JS rewriting `PATH`. ## What Changed - `codex-rs/arg0` now uses `InstallContext::current().package_layout.path_dir` to prepend the package helper directory before any threads are created. - Package helper `PATH` setup is independent from the temporary arg0 alias setup, so `codex-path` is still added even if CODEX_HOME tempdir, lock, or symlink setup fails. - `codex-rs/install-context` detects the canonical package layout we ship: `bin/`, `codex-resources/`, and `codex-path/` next to `codex-package.json`. - Shell, local unified exec, and user-shell runtimes now record package `codex-path` prepends in `explicit_env_overrides`, matching the existing zsh-fork behavior so shell snapshots cannot restore over the package helper path. - Remote unified exec requests do not receive the local app-server package path overlay. - `codex-cli/bin/codex.js` no longer computes or overrides `PATH`; it only locates the native binary in the canonical package layout and passes npm/bun management metadata. - Added regression tests for `PATH` ordering, package layout detection, and shell snapshot preservation of package path prepends. ## Verification - `node --check codex-cli/bin/codex.js` - `just test -p codex-install-context -p codex-arg0` - `just test -p codex-core user_shell_snapshot_preserves_package_path_prepend` - `just test -p codex-core tools::runtimes::tests` - `just bazel-lock-update` - `just bazel-lock-check` - `just fix -p codex-install-context -p codex-arg0 -p codex-core`

Wire managed MITM CA trust into child env (#22668)
## Stack 1. Parent PR: #18240 uses named MITM permissions config. 2. This PR wires managed MITM CA trust into spawned child processes. ## Why When Codex terminates HTTPS for limited mode or MITM hooks, child HTTPS clients need to trust Codex's managed MITM CA. Exporting proxy URLs alone is not enough, but blindly replacing user CA settings would be wrong: it can break custom enterprise/test roots, leak unreadable CA files into generated bundles, or make the child env disagree with its sandbox policy. ## Summary 1. Build immutable managed CA bundles under `$CODEX_HOME/proxy` that include native roots, the managed MITM CA, and only inherited or command-scoped CA bundles the child is allowed to read. 2. Export curated CA env vars alongside managed proxy env vars while preserving user CA override semantics, including nested Codex `SSL_CERT_FILE` precedence. 3. Thread generated CA bundle paths into child sandbox readable roots, including debug sandbox execution, so the exported env vars work inside sandboxed commands. 4. Remove only Codex-generated MITM CA bundle env when a child intentionally drops managed proxying for escalation or no-proxy retry. 5. Document the managed CA bundle behavior and cover env injection, per-child bundle generation, sandbox readable roots, and no-proxy cleanup in tests. ## Validation 1. Ran `just test -p codex-network-proxy`. 2. Ran `just test -p codex-protocol`. 3. Ran `just fix -p codex-network-proxy -p codex-protocol`. 4. [codex] Consolidate shared prompts in codex-prompts (#25151)
## Why `codex_core` is consistently a bottleneck for incremental builds during iteration. The simplest fix is to make the crate smaller. ## Summary `codex-core` owns several reusable prompt renderers and static prompt assets, which makes the crate harder to split apart. Rename `codex-review-prompts` to `codex-prompts` and move shared review, goal, permissions, compaction, realtime, hierarchical AGENTS.md, and `apply_patch` prompts into it. Move prompt-only tests and update consumers and `CODEOWNERS`. ## Validation - `just test -p codex-prompts -p codex-apply-patch` - `just test -p codex-core prompt_caching` - Bazel builds for the affected crates

windows-sandbox: pass workspace roots to runner (#24108)
## Why #23813 switches the Windows sandbox runner path to `PermissionProfile`, but it still left one runtime anchor for resolving symbolic `:workspace_roots` entries. That is not enough once a turn has multiple effective workspace roots: exact entries and deny globs under `:workspace_roots` need to be materialized for every runtime root before the command runner chooses token mode or builds ACL plans. ## What Changed - Replaces the Windows runner/setup `permission_profile_cwd` plumbing with `workspace_roots: Vec<AbsolutePathBuf>`. - Resolves Windows-local `PermissionProfile` data with `materialize_project_roots_with_workspace_roots(...)` instead of the single-cwd helper. - Threads `Config::effective_workspace_roots()` through core execution, unified exec, TUI setup/read-grant flows, app-server setup, app-server `command/exec`, and `debug sandbox` on Windows. - Preserves those workspace roots through the zsh-fork escalation executor instead of rebuilding them from `sandbox_policy_cwd`. - Makes `ExecRequest::new(...)` and the remaining `build_exec_request(...)` helper path take `windows_sandbox_workspace_roots` explicitly so new call sites cannot silently fall back to `vec![cwd]`. - Clarifies the `debug sandbox` non-Windows comment: remaining cwd-dependent resolution still uses `sandbox_policy_cwd`, while `:workspace_roots` entries are already materialized from config roots. - Updates elevated runner IPC `SpawnRequest` to send `workspace_roots` and bumps the framed IPC protocol version to `3` for the payload shape change. - Adds Windows-local resolver coverage for expanding exact and glob `:workspace_roots` entries across multiple roots, plus core helper coverage proving explicit roots are preserved. ## Verification - `cargo check -p codex-windows-sandbox -p codex-core -p codex-tui -p codex-cli -p codex-app-server` - `cargo test -p codex-windows-sandbox` - `cargo test -p codex-core windows_sandbox` - `cargo test -p codex-core unix_escalation` - `cargo test -p codex-app-server windows_sandbox` - `cargo test -p codex-tui windows_sandbox` - `cargo test -p codex-cli debug_sandbox` - `just test -p codex-core unified_exec` - `just test -p codex-core build_exec_request_preserves_windows_workspace_roots` - `env -u CODEX_NETWORK_PROXY_ACTIVE -u CODEX_NETWORK_ALLOW_LOCAL_BINDING just test -p codex-app-server --lib command_exec` - `just test -p codex-windows-sandbox` - `just test -p codex-exec sandbox` - `just fix -p codex-core -p codex-app-server -p codex-windows-sandbox` A local macOS cross-check with `cargo check --target x86_64-pc-windows-msvc ...` did not reach crate Rust code because native dependencies require Windows SDK headers (`windows.h` / `assert.h`) in this environment; Windows CI remains the real target validation. Two local targeted filters compile but do not run assertions on macOS: `env -u CODEX_NETWORK_PROXY_ACTIVE -u CODEX_NETWORK_ALLOW_LOCAL_BINDING just test -p codex-app-server --lib command_exec_processor` matched zero tests, and `just test -p codex-linux-sandbox landlock` matched zero tests because the landlock suite is Linux-only.

Wire task completion into thread-idle lifecycle (#24928)
## Why #24744 introduced the thread idle lifecycle hook so idle continuation can be owned by lifecycle contributors instead of hard-coded goal runtime plumbing. Task completion still called `goal_runtime_apply(GoalRuntimeEvent::MaybeContinueIfIdle)` directly, so the post-turn idle transition remained goal-specific and did not notify generic thread lifecycle contributors. ## What Changed - Add `Session::emit_thread_idle_lifecycle_if_idle()` to gate idle emission on both no active turn and no queued trigger-turn mailbox work. - Call that helper when a task clears the active turn, replacing the direct `GoalRuntimeEvent::MaybeContinueIfIdle` path. - Cover the behavior with `codex-core` session tests for emitting after task completion and suppressing idle emission while trigger-turn mailbox work is pending. ## Verification - New tests in `core/src/session/tests.rs` exercise the idle lifecycle emission and trigger-turn mailbox guard.

Add experimental turn additional context (#24154)
## Summary Adds experimental `additionalContext` support to `turn/start` and `turn/steer` so clients can provide ephemeral external context, such as browser or automation state, without turning that plumbing into a visible user prompt or triggering user-prompt lifecycle behavior. ## API Shape The parameter shape is: ```ts additionalContext?: Record<string, { value: string kind: "untrusted" | "application" }> | null ``` Example: ```json { "additionalContext": { "browser_info": { "value": "Active tab is CI failures.", "kind": "untrusted" }, "automation_info": { "value": "CI rerun is in progress.", "kind": "application" } } } ``` The keys are opaque and caller-defined. ## Context Injection When provided, accepted entries are inserted into model context as hidden contextual message items, not as visible thread user-message items. `kind: "untrusted"` entries are inserted with role `user`: ```text <external_${key}>${value}</external_${key}> ``` `kind: "application"` entries are inserted with role `developer`: ```text <${key}>${value}</${key}> ``` Values are not escaped. Each value is truncated to 1k approximate tokens before wrapping. For `turn/start`, accepted additional context is inserted before normal user input. For `turn/steer`, additional context is merged only when the steer includes non-empty user input; context-only steers still reject as empty input. ## Dedupe Strategy `AdditionalContextStore` lives on session state and stores the latest complete additional-context map. Each `turn/start` or non-empty `turn/steer` treats its `additionalContext` as the current complete set of values. Entries are injected only when the key is new or the exact entry for that key changed, including `value` or `kind`. After merging, the store is replaced with the provided map, so omitted keys are removed from the retained set and can be injected again later if reintroduced. ## Why Pending model input was split across `Session`, `TurnState`, and the agent mailbox. That made it easy for new paths to manage queued user input or mailbox delivery outside the intended ownership boundary. goals: keep pause transitions explicit (#23088)
## Problem This addresses several user-reported cases where active goals were paused even though the user had not explicitly asked for that transition: - the guardian approval-review circuit breaker interrupted a turn and implicitly paused the goal - a shutdown in one app-server instance could pause a goal while a second instance was still actively running the same thread - steering-style interrupts could also pause the goal even though they are meant to redirect work, not stop the goal lifecycle The common problem was that core treated `TurnAbortReason::Interrupted` as an implicit request to transition the persisted goal to `paused`. That made unrelated interrupt paths mutate goal state as a side effect, and in the multi-app-server case it allowed stale process teardown to pause a live goal owned by another running client. After this change, transitioning a goal to `paused` is always an explicit action performed by a client or another intentional goal-state mutation. It is never an implicit transition triggered by generic interrupt handling. Refs #22884. ## What changed - Remove the goal runtime path that paused active goals after interrupted task aborts. - Drop the now-unused abort reason from `GoalRuntimeEvent::TaskAborted`. - Update the focused regression coverage so an interrupted active goal still accounts usage but remains `active`.

Make extension lifecycle hooks async (#23291)
## Why Extension lifecycle hooks sit on the host/extension boundary, but the current trait surface only allows synchronous callbacks. That forces extensions that need to seed, rehydrate, observe, or flush extension-owned state during thread and turn transitions to either block inside the callback or move async work into separate host plumbing. Remove no-tool goal continuation suppression (#20523)
## Why `/goal` is supposed to keep Codex working until the goal is actually done. The previous continuation logic had two ways to stop early: the continuation prompt told the model to wait for new input when it felt blocked, and the runtime suppressed another continuation turn after a continuation finished without any tool calls. That made goals stop short even when the agent could still keep making progress (I received a few reports of this from users). It also relied on a brittle heuristic that treated "no registry tool calls" as equivalent to "should stop." ## What changed - removed the continuation prompt sentence that told the model to stop and wait for new input when it could not continue productively - removed the goal runtime suppression heuristic that stopped auto-continuation after a no-tool continuation turn - deleted the continuation-activity bookkeeping and left `tool_calls` as telemetry only - added focused regressions for the two intended behaviors: completed no-tool continuation turns still continue, while `request_user_input` keeps the existing turn open instead of spawning a new continuation

Add turn start timestamp to turn metadata (#19473)
## Why - Without change: MCP tool calls receive `_meta["x-codex-turn-metadata"]` with `session_id` and `turn_id`. - Issue: MCP servers may want the turn start timestamp to measure internal latency relative to turn start. ## What Changed - With change: turn metadata now includes `turn_started_at_unix_ms`, which is propagated to MCP tool calls in `_meta["x-codex-turn-metadata"]`. ## Verification - `codex-rs/core/src/mcp_tool_call_tests.rs` - `codex-rs/core/src/turn_metadata_tests.rs` - `codex-rs/core/src/turn_timing_tests.rs` - `codex-rs/core/tests/responses_headers.rs` - `codex-rs/core/tests/suite/search_tool.rs`

Remove ghost snapshots (#19481)
## Summary - Remove `ghost_snapshot` / `GhostCommit` from the Responses API surface and generated SDK/schema artifacts. - Keep legacy config loading compatible, but make undo a no-op that reports the feature is unavailable. - Clean up core history, compaction, telemetry, rollout, and tests to stop carrying ghost snapshot items. ## Testing - Unit tests passed for `codex-protocol`, `codex-core` targeted undo and compaction flows, `codex-rollout`, and `codex-app-server-protocol`. - Regenerated config and app-server schemas plus Python SDK artifacts and verified they match the checked-in outputs.

permissions: derive legacy exec policies at boundaries (#19737)
## Why After config and requirements store canonical profiles, exec requests should not cache a derived `SandboxPolicy`. The cached legacy value can drift from the richer profile state, and most execution paths already have the filesystem and network runtime policies they need. ## What Changed - Removes `sandbox_policy` from `codex_sandboxing::SandboxExecRequest` and `codex_core::sandboxing::ExecRequest`. - Adds an on-demand `ExecRequest::compatibility_sandbox_policy()` helper for the Windows and legacy call sites that still need a `SandboxPolicy` projection. - Updates Windows filesystem override setup and unified exec policy serialization to derive that compatibility policy at the boundary. - Updates Unix escalation reruns and direct shell requests to reconstruct exec requests from `PermissionProfile` plus runtime filesystem/network policy, without carrying a cached legacy policy. - Adjusts sandboxing manager tests to assert the effective profile rather than the removed legacy field. ## Verification - `cargo check -p codex-config -p codex-core -p codex-sandboxing -p codex-app-server -p codex-cli -p codex-tui` - `cargo test -p codex-sandboxing manager` - `cargo test -p codex-core exec_server_params_use_env_policy_overlay_contract` - `cargo test -p codex-core unix_escalation` - `cargo test -p codex-core exec::tests` - `cargo test -p codex-core sandboxing::tests`

permissions: migrate approval and sandbox consumers to profiles (#19393)
## Why Runtime decisions should not infer permissions from the lossy legacy sandbox projection once `PermissionProfile` is available. permissions: make runtime config profile-backed (#19606)
## Why This supersedes #19391. During stack repair, GitHub marked #19391 as merged into a temporary stack branch rather than into `main`, so the runtime-config change needed a fresh PR. `PermissionProfile` is now the canonical permissions shape after #19231 because it can distinguish `Managed`, `Disabled`, and `External` enforcement while also carrying filesystem rules that legacy `SandboxPolicy` cannot represent cleanly. Core config and session state still needed to accept profile-backed permissions without forcing every profile through the strict legacy bridge, which rejected valid runtime profiles such as direct write roots. The unrelated CI/test hardening that previously rode along with this PR has been split into #19683 so this PR stays focused on the permissions model migration. ## What Changed - Adds `Permissions.permission_profile` and `SessionConfiguration.permission_profile` as constrained runtime state, while keeping `sandbox_policy` as a legacy compatibility projection. - Introduces profile setters that keep `PermissionProfile`, split filesystem/network policies, and legacy `SandboxPolicy` projections synchronized. - Uses a compatibility projection for requirement checks and legacy consumers instead of rejecting profiles that cannot round-trip through `SandboxPolicy` exactly. - Updates config loading, config overrides, session updates, turn context plumbing, prompt permission text, sandbox tags, and exec request construction to carry profile-backed runtime permissions. - Preserves configured deny-read entries and `glob_scan_max_depth` when command/session profiles are narrowed. - Adds `PermissionProfile::read_only()` and `PermissionProfile::workspace_write()` presets that match legacy defaults. ## Verification - `cargo test -p codex-core direct_write_roots` - `cargo test -p codex-core runtime_roots_to_legacy_projection` - `cargo test -p codex-app-server requested_permissions_trust_project_uses_permission_profile_intent` --- [//]: # (BEGIN SAPLING FOOTER) Stack created with [Sapling](https://sapling-scm.com).
